Community Outreach Assistance With The Health Insurance Marketplace

As a community health center and a certified patient centered medical home, Georgia Highlands Medical Services provides several additional ancillary and support services to ensure every patient receives the support they need to achieve the very best health outcome. Some examples of the support services we provide our patients include:

Case Management & Eligibility Assistance

Georgia Highlands Medical Services offers help to patients connecting to other resources and benefits, including access to food, housing, legal and other assistance. In addition, Georgia Highlands is an Assisted Service Site in the Georgia Gateway program. As a Georgia Gateway partner, GHMS has access to a one-stop-shop web-based system that allows staff to help patients and community members with submission of Medicaid, SNAP, Child Care, WIC and other benefits.

Health Education

Through our Health Education Program, we teach our patients with their entire well-being in mind through physical, mental and social health education. At a minimum, these services include education regarding the availability and appropriate use of health services and also include primary prevention and/or targeted education on self-managed care and other health promoting behaviors for patients with identified risk factors or conditions, including things like smoking cessation, nutrition, healthy choices and management of specific chronic conditions.

Care Coordination / Chronic Disease Management

We provide comprehensive patient care coordination and chronic disease management services through a care team approach. We provide self-management support to each patient, working with each to help the patients become better managers of their own health. It’s a collaborative process that empowers patients to take better care of themselves by setting realistic goals and action plans to reach them. Likewise, we provide the kind of care coordination that ensures our patients get what they need when it is needed. At Georgia Highlands, we are a system of care in partnership with hospitals, service organizations and others across the region to ensure a seamless continuity for the patients who call us their medical home.
