Welcome

At Georgia Highlands Medical Services, we work in teams. You are not only a member, but at the center of that team. The Georgia Highlands team members consist of a doctor or nurse practitioner, medical assistant, clinical care coordinator, laboratory technician, front office receptionist, and interpreter, as needed. This way, you will get to know your health care team, they know you, and, together, we work together to improve your health.

Our in-house services include family practice, pediatrics, geriatrics, women’s health, diagnostic laboratory, interpreter services, testing and screenings, family planning, immunizations, referral to specialty services, in-house pharmacy, care management, health education, and community outreach. Our providers follow evidence-based practices with guidelines and recommendations from ADA, AHA, ACIP, ATP3, USPSTF, AAP, AAFP, APA, ACP, Up to Date and Bright Futures.

We offer a Sliding Fee / discount program that allows individuals with no insurance or a low income to receive medical services, lab orders and pharmacy at a discounted rate based on income status and household size. Through this program, fees can be reduced or “slide”. Age, sex, race, creed, sexual orientation, disability, national origin, or legal presence/status are not considered. To qualify, you must complete an application and provide proof of documentation. An approved application is good for 12 months and will expire then.

ELIGIBILITY IS BASED ON HOUSEHOLD INCOME AND SIZE

Household Income
Family Household Income includes all income from all family members, including Employment Income, Self-employed Income, and income from non-employment sources such as Social Security, Disability, Alimony, Child Support, Retirement, and Unearned Income. The most common proof of income used is paystubs. For a detailed list of acceptable forms, please consult any front office staff member at our Georgia Highlands Medical Services location.

Household Size
A Family or household can consist of one person, a group of people, or one or more families living or staying temporarily. The members do not have to be related by blood or marriage. Individuals include parents, foster children, distant relatives, and/or guests staying longer than 30 calendar days.

We recognize that every situation is unique, and our trained staff at Georgia Highlands Medical Services are here to assist you with your application and ensure that the correct documents are submitted. If you have any questions or need guidance, please do not hesitate to contact us.

Sliding Fee Discount Program

You may be eligible to participate in our Sliding Fee program. If so, you can pay for services based on the size of your family and your family income. If you believe you may be eligible, you will need to complete a sliding fee application and will be required to present a picture ID and proof of household income. Staff will be able to assist you at any of our locations and will help make sure that you have the right documents.
