Unsure of what to expect on your first visit to Georgia Highlands?  Let us take the stress out of getting ready for that first appointment with your new provider.

You will be given a Notice of Privacy Practices, and be asked to sign a Patient Consent Form.

Prior to your visit, please download, print and complete the Patient Information Sheet and a copy of our privacy practices. Click the button below to access the forms.
You will present your photo identification and insurance card for verification.  If you do not have health insurance, or if your current insurance is invalid, you will be asked to meet with an intake staff person who will further assist you.  The intake staff will give you information about GHMS and copies of the Patients' Bill of Rights and Responsibilities as well as Advance Directives.  Please also bring a list of medications you are currently taking, both prescription and over the counter, as well as your payment for the visit.  If you are applying for the sliding scale, please bring in approved proof of income and household size.  The receptionist will assist you in completing the registration process. afterwards, you may take a seat in the waiting area until the Medical Assistant calls you back.

After you have been seen by the provider, the Medical Assistant will assure that you receive all your prescriptions, referrals, and follow up appointments.

Georgia Highlands Medical Services offers sliding fee scale discounts for all patients who are financially eligible.  Please call our office during business hours for details.
